The Exercise Referral scheme is a partnership between South Derbyshire District Council, Derbyshire County Primary Care Trust and Greenbank Leisure Centre.
The aims of the Scheme are:
- To improve the standard of health for individuals who have been recommended as being suitable for developing their fitness/activity levels and general well being through appropriate regular exercise
- To encourage people to take greater responsibility in improving their own health by pursuing a more physically active lifestyle
Who can access the Scheme?
To have access to the scheme you must visit your surgery and ask a member of the Health Care Team to refer you using the appropriate referral form
Who can refer me to the Scheme?
Any Health Practitioner at participating surgeries such as a GP, Practice Nurse or District Nurse, Health Visitor, Physiotherapist etc.
Do I need a reason to be referred?
There are criteria for you to be referred to the scheme, these are:
- risk of Coronary Heart Disease (CHD)
- stroke (CVA)
- high Blood Pressure
- diabetes (type 1 & 2)
- chronic Obstructive Pulmonary Disease (COPD) / Asthma
- osteoarthritis
- osteoporosis
- weight reduction for a body mass index 30 plus
- social / Mental Health / Depression

How long will the Scheme last for?
The Scheme is over a 12 week period. You will see a fitness instructor at the start to discuss your physical activity likes and dislikes and jointly agree a realistic and achievable activity plan. Any medical conditions are discussed and appropriate forms completed. A development record is issued for you to record your activity levels. You will be given a programme of activity leaflet where you can attend group sessions and supported supervised classes taken by staff at leisure facilities or attend any of the individual activities on offer
What happens when the Scheme ends?
At the end of the 12 weeks, any changes in activity levels, body measurements, confidence, motivation, will be recorded. You can discuss what to do next with the fitness instructor and will be encouraged to maintain any activities you were doing throughout the Scheme. A brief report will be submitted to your GP practice to inform them of completion and progress.
How much does it cost?
Only £5 for the initial gym induction which includes a health check/lifestyle questionnaire and setting of a physical activity plan. Each session is then only £2 and 12 sessions are scheduled as part of your programme.
